*Out-of-Network: Breaking Cycles Counseling encourages clients to verify out-of-network benefits with their insurance providers. Verify the out-of-pocket costs, such as deductibles and out-of-pocket maximums required before full benefits are covered. A detailed invoice outlining the services received (Super Bill) can be provided to the client. The client can submit the Super Bill to their insurance provider for full or partial reimbursement. 

No Show/ Late Cancellation Policy: Breaking Cycles Counseling requires the client to cancel 48 hours before the scheduled appointment. Breaking Cycles Counseling will charge the fee for an entire session for any No-Show and Late Cancellation. Please note that if the client's cancellation is at least 48 hours in advance, you will not incur a No-Show/ Late cancellation fee. 

If two (2) appointments are missed without notifying the office, the regularly scheduled appointments will be given to someone else, and any remaining appointments will be canceled. At this point, you may be provided with a list of other providers near the area and/ or asked to contact your insurance carrier. 

Letters: Breaking Cycles Counseling does not provide disability letters, emotional support animal letters, immigration letters, letters regarding your ability to work, or any letters that would inform providers of your mental health history. These needs must be discussed with your medical/ primary care provider. Letters confirming the client's participation in therapy will be provided to actively participating clients. These letters do not provide additional information besides the client participating in therapy sessions. Request for letters may result in additional fees.
